Guide to Completing you Sign up process

We are very keen to assist you to start the business - share with you about the products, answering any questions that you may have and just sharing our experiences with you. We'll do that very soon. However, first things first..
The Singapore ForLife office requires that the paper application are submitted, and so please take a moment to download and fill in the forms - The Distributor Application form.
Singapore Distributor Application Form
Congratulations on signing up to become a Distributor with 4Life. The company requires hard copies to complete the sign up process. Here are the guidlines, the download links, and the fax number to fax to.
1) Fill in your name, address, contact details,etc
2) Enroller information: Fill in as > : Yeap Poh Yeim, id number: 6784120
3) Sponsor Information: Fill in as>Yeap Poh Yeim, id number: 6784120
Ignore the part about W-8
4) Sign the form.
1) Fill in your name etc, and where you wish the products to be shipped to.
2) Fill in the product you wish to buy now. (Don't worry about the "item number" )
3) Fill in your credit card details.
We encourage you to be on autoship because its many benefits - so consider to be on this. Autoship is like a subscription, - like you subscribe to a favorite magazine, or subscribe to cable TV etc.
